When acute care hospitalization is no longer necessary, but a patient is not physically able to function at home, TCU is a positive intermediary step for rehabilitation. Transitional Care is needed in these situations...

  • Recovery from major surgery, joint replacement, vascular or abdominal procedures
  • Repeated hospitalization
  • Recovery after major accident or stroke
  • Pain Management
  • Wound Management for injuries which are not healing or need special care
  • Nutritional Therapy including tube feeding
  • Chronic Disease Management such as Parkinson’s
  • IV Therapy
