Long Term Care Social Services

Medical services are only one facet of caring for our aging population. Our Social Services department is available to meet both the daily needs of our residents and their families on subjects like adjusting to the environment, relationships and emotional needs.

 

Our Social Services professionals meet both initially and quarterly with residents and their families to complete a comprehensive psychosocial assessment of attitudes and needs. This information is integrated into the resident’s plan of care. Our staff cares about the whole person, and information from this assessment helps us give a higher, more personal quality of care.

 

Colleen and her staff also seek knowledge of community resources in order to match people with special needs to appropriate resources, and they serve as discharge planners to assist residents and family members in exploring various alternative living arrangements, such as
Transitional Care Rehabilitation Unit and Assisted Living environments.
