Patient Information

Before you arrive…  When making your initial appointment, make sure you have your physician referral in place. If you don’t know if your physician has referred you to Lincoln Physical Therapy, call and ask him or her to do so. You should also double check your insurance coverage to see if services will be covered.

When to arrive…  We’re here Monday through Saturday. Unless there is an emergency, you will not be kept waiting more than ten minutes for your appointment, but please allow yourself 45 minutes to an hour for each physical therapy session.

We ask all patients to arrive 15 minutes early for their first appointment. This will give us ample time to do necessary paperwork, leaving your scheduled appointment time to focus on your health needs.

Cancellations…  Please notify us 24 hours in advance when you need to cancel or reschedule your set appointment.

What to wear…  If you are being seen for a condition involving your neck, shoulders or upper limbs, please wear attire that allows easy access to your neck and arms. Women are recommended to bring a sports bra or tank top.

If you are being seen for a condition involving your lower back or legs, please wear short pants or athletic shorts. Also, please wear or bring the shoes that you most commonly use during day-time hours.

What you can expect during my initial physical therapy visit...

People often ask this question. Your initial visit will begin with a thorough medical history followed by a comprehensive examination and assessment of your physical condition. From the assessment findings, we will design a customized treatment plan to meet your specific needs. Treatment may include:

  • Therapeutic exercises
  • Modalities to relieve pain and encourage healing (electrical stimulation and/or ultrasound, among others)
  • Hands on manual therapy to relieve pain and encourage joint and muscle flexibility
  • Movement and/or posture retraining
  • Therapeutic activities to assist in your safe return to customary physical activities.

We also emphasize the use of an individualized home program, so that you can take the initiative for your recovery between appointments.

Physical Therapy Insurance Information

Physical therapy services are available to any person who is under the care of a physician or other primary medical provider. Please tell us about your coverage on, or before, your first visit.

Referrals

Rehabilitation services are covered by most insurance providers as long as you have preauthorization and a physician referral -- a written prescription from your doctor or other provider dated during the previous 30 days. Please confirm your referral with your primary care physician prior to your initial appointment at Lincoln Physical Therapy.

If you are not sure whether your primary care physician has given you a referral to Lincoln Physical Therapy, contact your physician and ask for a referral or call us.

Insurance

Again, please tell us about your coverage on, or before, your first visit. We’ll be glad to work with you and bill your medical insurance directly if you wish. Any time there is any change in your coverage, please let us know to prevent unnecessary misunderstandings or unnecessary delays. If you're having trouble with your insurance company, contact us. We will be happy to assist you in any way we can.

Without Insurance

Talk with us ahead of time about payment arrangements. If you are not covered by insurance, we will be happy to set up a payment plan.

Cost for medical equipment

Your particular insurance coverage will determine whether or not payment for equipment is included in your plan.

Other options

Our primary concern is for your health and general well-being. We don’t want your recovery complicated or delayed because of payments concerns. Let’s talk about your needs then, together we will find a suitable payment arrangement for you.
